I am an in-network provider for Aetna PPO/POS and Blue Cross Blue Shield PPO plans.
Prior to your first appointment, please contact your insurance company directly to inquire about the following information:
Is Dr. Fortunato in my network?
What are my outpatient mental health benefits for in-office and/or telehealth visits?
Do my benefits cover individual and/or couples therapy?
Do I have a copay, deductible, and/or coinsurance? If I have a deductible, has it been met?
Do I have a specific number of sessions that are covered each year?
I am considered an out-of-network provider for all other insurance plans. Should you wish to use your out-of-network benefits, I am happy to provide you with documentation that you can submit to your insurance company for possible reimbursement.
Prior to your first appointment, you will be asked to complete initial paperwork. This can be done online through a secure client portal. If you do not have internet/computer access, please contact Dr. Fortunato, and alternate arrangements will be made.
Payment is due at the time of service. I am able to accept debit/credit/HSA/FSA card payments.
I request 24 hours notice to change or cancel an appointment. With less than 24 hours notice, you will be responsible for the full cost of your appointment.
